A Clinic in Rural Colorado?
By the end of the initiative, CCR should have:
Established a sustainable healthcare operation serving Park County.
Ultimately established or facilitated the establishment of a physical healthcare clinic in Park County.
Connected at least 3,000 Coloradans with healthcare services or healthcare access.
Created new healthcare-related employment opportunities.
Created pathways for healthcare education, training, internships, or workforce development.
Developed services that improve access for Park County seniors, including Medicare beneficiaries.
Determined whether an unskilled home-care operation can sustainably support the broader healthcare mission.
Established partnerships with healthcare providers, local government, emergency services, educational institutions, insurers, Medicare/Medicaid-related resources, and community organizations.
That’s right, with the help of our Healthcare Committee and numerous partners with backgrounds as nurses, physicians, and providers, we are undertaking a project to establish a clinic in Park County, Colorado.

CCR is preparing to launch a new medical resources LLC and has increased its partnerships for the Colorado Rural Health Initiative.
-
Identify businesses, nonprofits, charities, and individuals that can be of help with the initiative.
-
CCR will establish a statewide referral network and watchdog service connecting people in urgent need of health and community assistance with trusted nonprofit organizations, beginning with targeted providers in Denver, Greeley, Loveland, Colorado Springs, and surrounding communities.
-
CCR will pilot a Park County home-care program providing affordable, non-medical in-home assistance, with the first 90 days focused on legal, insurance, staffing, pricing, client, caregiver, and reimbursement feasibility, and a first-year goal of serving 25–50 residents to determine whether the program should expand.
-
CCR will conduct an expanded planning and research phase for the proposed Park County clinic, focusing on funding, legal and operational requirements, location, pricing and financial sustainability, strategic partnerships, required assets, and the clinic’s ability to operate independently beyond initial CCR investment.
Project Phases
-
CCR will begin establishing the Park County clinic by securing funding and required assets, filing formation documents, obtaining necessary licenses and certifications, establishing its official ownership and management structure, and acquiring the materials needed for operations.
-
CCR will launch and continuously improve the Park County clinic by securing and preparing the property, observing operations, identifying and evaluating challenges, implementing solutions, and repeatedly applying the OODALOOP process to improve the clinic’s performance.
-
CCR will evaluate the clinic’s long-term sustainability after establishment, identify potential future challenges, and implement mitigation strategies to strengthen the clinic’s ability to remain operational and financially stable.
-
CCR will evaluate the clinic’s success, identify opportunities to expand into other rural Colorado communities, and replicate Phases 4–7 to establish additional clinics where appropriate.
